What is the Pack Health program?

The program is 12 weeks, the health advisor calls weekly to discuss progress toward a goal. The call is more successful when the patient has completed the digital lesson.

They determined that tiny steps were crucial. Patients set a tiny step each week. They tended to do more of something, rather than reduce something. They discuss their tiny steps with their health advisors, set them up, celebrate the wins, and push through discomfort when they don’t meet their goals.

Members want to track their tiny steps. So the team created a member web app for setting, tracking, and reaching tiny steps. The engagement improves health outcomes. In one study, 95% of members maintained or improved their blood pressure.

They found that nudging worked, the human connection was a driver, but the digital engagement is what made it successful.
